To sign up for Health Insights, you need to live in the US, in a state where Health Insights is available.
You can sign up if you live in:

- Maine
- Minnesota
- Nebraska
- New Hampshire
- New Jersey
- New Mexico
- North Dakota
- Ohio
- South Carolina
- Vermont
- West Virginia
We ask for your home address and shipping address when you sign up. If your address is outside of one of these states, we cannot offer Health Insights to you at this time.
We cannot offer Health Insights if your address is on a military base or in a US territory (American Samoa, Guam, the Northern Mariana Islands, Puerto Rico and the US Virgin Islands).
